Print the membership application form, include the appropriate completed and signed waiver(s) and payment, and send to the address on the form.
These documents are "fillable forms" which may be filled out with data by typing with your computer keyboard, before printing and mailing. This will save you manual handwriting, and will avoid any problems with the membership director having difficulty reading your handwriting.
